KD is thought to result from a combination of vascular, anatomic and traumatic insults resulting in interruption of the blood supply to the lunate and osteonecrosis. A 58-year-old retired engineer presented with a 20-year history of right wrist pain, swelling and decreased function. Examination revealed tenderness over the dorsal lunate, decreased wrist flexion-extension arc and decreased grip strength. KD is more common in men and rarely affects bilateral wrists. The symptoms of KD are insidious and easily mistaken for a minor wrist injury. Early disease may appear normal on plain radiographs with pathological change only evident on MRI. Our patient’s radiographs demonstrated a sclerotic fragmented lunate with loss of lunate height (figure 1) and CT imaging determined that the radiocarpal and intercarpal joints were relatively well preserved (figure 2).

A comparative study of the surgical procedures to treat advanced Kienböck's disease.

We have treated a total of 16 cases of advanced Kienböck's disease, stage III and IV by Lichtman's classification, with triscaphe fusion, tendon ball replacement arthroplasty after excision of lunate, proximal row carpectomy as a salvage procedure and limited wrist fusion, since 1985. All cases were followed for minimal 16 months after each operation. Tendon ball replacement arthroplasty after ...

Distal capitate shortening with capitometacarpal fusion for management of the early stages of Kienböck’s disease with neutral ulnar variance: case series

BACKGROUND The aim of surgical management of Kienböck's disease has been proposed to slow the progressive osteonecrosis and secondary carpal damage. The aim of this case series was to evaluate the results of a new technique, combining distal capitate shortening with capitometacarpal fusion for the treatment of Kienböck's disease (Lichtman stage II or stage IIIA) in neutral ulnar variance patien...

The incidence of asymptomatic Kienböck's disease.

The aim of this study was to determine the incidence of asymptomatic Kienböck's disease in patients who attended the Dr George Mukhari Hospital (formerly Ga-Rankuwa Hospital), as well as the relevance of ulnar variance on the disease. This was a retrospective study. In a 12 month period we reviewed postero-anterior radiographs of 1287 patients seen at our radiology department, with complaints u...
